Essay

The dissertation is devoted to the problem of improving the results of diagnostics and treatment of patients with bullous pulmonary emphysema, complicated by spontaneous pneumothorax, by developing new types of videothoracoscopic interventions and creating appropriate algorithms for surgical tactics. The paper presents the results of studies of 423 patients with various forms of bullous emphysema complicated by spontaneous pneumothorax, who were treated in the emergency surgery, traumatic shock, military surgery with emergency surgery of the State Institution by the name of V.T. Zaitsev of the National Academy of Medical Sciences of Ukraine from 2006 to 2019. Complex treatment of patients with bullous emphysema has summarized 2 stages of surgical care. The purpose of the first stage was to objectify the etiology of the disease, which was used for videothoracoscopic biopsy, followed by pathomorphological examination, the results of which were calculated by DI, determined the risk of pneumothorax recurrence and the form of bullous emphysema. The purpose of stage 2 was to develop surgical operations aimed at radical correction of bullous emphysema based on objective data on the form of the disease. The analysis of the immediate and long-term results of treatment of patients with bullous emphysema, complicated by spontaneous pneumothorax, in the studied groups conclusively proves that the use of the developed surgical interventions made it possible to significantly reduce the total number of complications.
