Essay

The dissertation provides a new solution to a scientific problem, which consists in improving methods for the prevention, diagnosis and treatment of perimenopausal disorders in order to improve the quality of women’s life over 40 at the age of their greatest professional and social activity. Based on the data of longitudinal observation of patients in the perimenopausal period for 10 years, the main factors and predictors of early occurrence and developmental features and severity of menopause syndrome were identified. The revealed data of gynecological and reproductive history, extragenital pathology, features of professional and social life, psychological characteristics of the personality, as well as awareness of perimenopausal disorders and a woman’s desire to use modern methods of treatment of climacteric syndrome once again confirm the conclusion about the need for early and timely correction of hormonal disorders to maintain health and quality of life in the future. The analysis of genetic, social, and biomedical factors served as the basis for creating a mathematical model for predicting the occurrence of early menopausal disorders, taking into account indicators of reproductive and biological age. The necessity of the early prescription of hormone replacement therapy for the correction of menopausal disorders using the capabilities of the "therapeutic window", not only for the treatment of severe menopausal syndrome, but also for the prevention of cardiovascular pathology, osteoporosis, severe psychoemotional disorders and depressive states is proved. The study of the level of awareness of the female population of Ukraine, as well as gynecologists and other specialists (cardiologists, neurologists and family physicians) regarding perimenopausal disorders and, accordingly, health risks associated with this period, once again emphasizes the need for a wide discussion of menopause issues both among the population and at specialized interdisciplinary scientific forums. A study of the main manifestations of climacteric syndrome, such as: severe psychoemotional, vegetative-vascular disorders, the development of genitourinary syndrome of menopause, as well as their connection to the hormonal status, allows us to conclude that the development, course and severity of menopause syndrome directly depends on the onset of menopause, it's duration and prescription of preventive and therapeutic modern methods of treatment. Individual, pathogenetically substantiated, differentiated with respect to the revealed violations, complexes of treatment and prophylactics for perimenopausal patients have been developed with mandatory consideration for the morphological and functional state of target organs of the sex steroid hormones, mammary glands and endometrium, with the aim of treating not only menopausal syndrome, but also preventing proliferative processes and oncopathology.
