Essay

The work is devoted to the study of the peculiarities of alcoholic cirrhosis of the liver (ALC) in combination with obesity and the search for effective treatments. The study found that risk factors for adverse of liver cirrhosis in combination with obesity are disorders of systemic hemodynamics with the development of cardiac remodeling, increasing with the progression of hepatic decompensation and accompanied by activation of systemic inflammatory syndrome, endothelial dysfunction (ED), dyslipidemia, adipocytokines imbalance, insulin resistance, increased endotoxemia, hemostasis violations and activation of fibrosing processes. Differentiated approaches to the treatment of patients with ADC in combination with obesity have been developed. To improve the functional state of the liver, metabolic disorders, reduce the manifestations of endogenous intoxication, systemic inflammatory syndrome and ED, reduce fibrosing processes and reduce the rate of ALC progression, it is recommended to prescribe ademethionine and arginine glutamate, for the correction of dyslipidemia – rosuvastatin, to improve the course of ALC classes B and C – a long course of albumin, for the correction of hyperaggregation states in patients with class A and non-valvular atrial fibrillation – rivaroxaban.
