The dissertation is devoted to the complex research of semiotic, syndromological and structural and dynamic differences of psychopathological disorders in persons with pathology of exogenous-organic genesis and its multimorbid combinations with states of exogenous-intoxication (infectious) genesis and endogenous-genesis. The study was based on the examination of 393 people, of whom 221 with the presence of mental pathology of exogenous organic (cerebrotraumatic) genesis without aggravation of diseases of another genesis and in various multimorbid combinations with psychopathology arising from exogenous intoxication (as a result of infectious diseases) or due to vascular diseases, 70 people with the presence of pathology of endogenous organic (vascular) genesis (arterial hypertension, atherosclerosis of the cerebral vessels) and without pathology of exogenous organic (cerebrotraumatic) or exogenous intoxication (infectious) genesis, 52 patients with infectious pathology (acute respiratory infections of the upper respiratory tract (acute rhinitis, acute sinusitis, acute pharyngitis, acute laryngitis, acute tonsillitis) and acute respiratory infections of the lower respiratory tract (bronchitis), which did not have an endogenous organic (vascular) and exogenous organic (cerebrotraumatic) burden nosity according to anamnestic information and 50 conditionally healthy persons. For realization of the purpose and tasks of the research, the complex of research methods was used in the work, which included: clinical-anamnestic, clinical, clinical-psychopathological, pathopsychological, catastrophic, functional-diagnostic, laboratory, statistical methods. The influence of conditions of exogenous-intoxication (infectious) and endogenous-organic (vascular) genesis on the structure and semiotic signs of psychopathological manifestations in persons with pathology of exogenous-organic (cerebro-traumatic) genesis was revealed. It was established that in persons with disorders of exogenous-organic (cerebro-traumatic) genesis from endogenous-organic (vascular) genesis, a key nosological unit was a disorder of personality and behavior, the main difference between the semiotic continuum of psychopathological symptoms, dysphoric arrangement of the circle of psychopathological symptoms. The dependence of psychopathological disorders of traumatic genesis on the immunopathological process, deviation of the concentration of melatonin and disorder of the rhythm of melatonin secretion, which correlated with severe insomnia, flattening of the daily temperature curve, disturbance of the biological rhythm «sleep-wakefulness», expression of psycho-sympathema, psychosis, was proved. The triggers of development and exacerbation of psychopathological disorders in the studied contingent and differences in the level of stress of the triggers was revealed. Mechanisms of pathogenesis have been determined and a prognostic model of psychopathological disorders in individuals with pathology of exogenous-organic (cerebro-traumatic) and in various comorbid combinations with pathology of exogenous-intoxication (infectious) and endogenous-organic (vascular) genesis was constructed. On the basis of the established features, a program of pathogenetically grounded differentiated therapy and psychorehabilitation was developed for persons with psychopathological disorders due to pathology of exogenously-organic (cerebro-traumatic) and in combination with pathology of endogenously-organic (vascular). According to the results of testing of the developed program, its high efficiency was proved.
