Shepet'ko Y. Choice of rational pylorus-retaining and pylorus-corrigating organs-retaining operations in urgent surgery of the complicated duodenal ulcers

Dissertation is devoted to the problem of pylorus-retaining direction in urgent surgery of the complicated duodenal ulcers. In experiments in the dogs deeper inhibition of gastric secretion stimulation by pentagastrin after pylorus-retaining duodenoplasty with SPV is proved in comparison with other kinds of vagotomies. The classifications of pylorus-retaining and pylorus-corrigating operations are offered. The new methods of pylorus-retaining operations are developed: an expanding combined duodenoplasty of stenosing bleeding and perforated ulcers, valve duodenoplasry. The new methods of vagotomies (subtotal SPV, "Express" method tool SPV), and also four new methods of operations of complicated parapapillar ulcers are developed. The algorithms of tactics and application of functionally favorable methods of organs-retaining operation of bleeding and perforated ulcers of duodenum are developed.
