Galych S. Microsurgical autotransplantation of the composite tissues complexes in hign active zones of the extremities

The thesis deals with the problem of surgical treatment of patients with tissue defects in hign active zones of of the extremities such as hand, foot, stumps of the extremities and big joints by means of microsurgical free flap transfer. To achieve high level results of such operations the experimental, anatomic and clinical investigations were performed. Some new aspects of microsurgical autotransplantation were learned out such as ischemia-reperfusion flap tissues injuries and the possibilities of their protection; the state of central and peripheral parts of delayed flaps in different terms after their elevation; structural hanges of the flaps accoding to recepient zone conditions; the abilities of flap skin reinnervation. The result of investigation become the base for devicing some new kinds of operations and approaches to the microsurgical autotransplantation. The clinical application of this devices made it possible to decrease flap necrosis complications on 8,99 % and achieve functional reabili tation in 90,32 % of patients
