The dissertation is devoted to the theoretical generalization and a new solution of the urgent medical and social problem - optimization of diagnostic evaluation of the effectiveness of surgical treatment of men with primary left varicocele in terms of predicting the normalization of their reproductive capacity by identifying interrelated disorders of testicular hemodynamics and laboratory immunological immunological. The dissertation presents a clinical and statistical analysis of the results of diagnosis and surgical treatment of 214 patients with left varicocele II-III centuries. aged 19 to 33 years with concomitant sub- or infertility. According to the results of the dissertation, new data on the pathogenetic mechanisms of fertility disorders in men with left varicocele were obtained. The high efficiency of a safe non-invasive diagnostic method - high-quality sonoelastography for predicting the probability of testicular tissue damage in varicocele has been proved. Prognostic markers of fertility potential in patients with left varicocele are substantiated by sonoelastogram of spermatic cord and testis, spermogram, pro- / antioxidant, L-arginine / NO, ion-transport and immune systems in various biological tissues of patients. It has been determined that men with left-sided varicocele, who, despite varicocelectomy, have elevated levels of proinflammatory cytokines and prooxidant enzymes, are a real threat of loss of fertility.
