The dissertation is devoted to the theoretical generalization and a new urgent problem - improvement of prevention and treatment of local inflammatory manifestations in the surgical treatment of impacted lower molars by optimizing a set of measures using aprotinin and fluctuating current. 90 patients with impacted lower third molar (IMTM), who underwent surgery of moderate and severe degree of difficulty and IMTM with complications of acute serous pericoronitis were examined and treated. New data on the effectiveness of local application of aprotinin, which had a protective effect on IAN and perineural tissues during traumatization were obtained. Experimental, clinical and laboratory studies have proven the effectiveness of of aprotinin application and extraoral aprotinin fluctuophoresis in reducing the clinical symptoms of acute inflammatory response caused by surgical trauma. The use of intraoral fluorophoresis of aprotinin in patients with acute serous pericoronaritis on the background of IMTM accelerated the elimination of local symptoms. For the first time, the content of neuron-specific enolase as a marker of the degree of damage of IAN during removing of impacted lower molars was determined.
