Essay

In the dissertation, a theoretical generalization and a clinical-laboratory solution to the topical issue of orthodontics are carried out - increasing the effectiveness of the prevention of dental caries, dental caries and periodontal tissue diseases and their treatment in children with sensorineural hearing deprivation by substantiating, developing and implementing a treatment algorithm of the combined use of various types of treatment and prevention measures Thus, on the basis of a clinical examination of 61 hearing-impaired patients aged 9-20 years, it was determined that the prevalence of hearing loss among children with hearing loss is 100%. The most common bite pathology in deaf children is Class I according to Angle (73.77%), second place is Class II according to Angle (26.23%). The infantile type of swallowing was determined in 39.34% of children, in 19.68% of DICs, oral breathing was observed, 80.33% of examined children with hearing impairments have anomalies of attachment of soft tissues of the oral cavity. Caries occurs in 77.05% of deaf children, the intensity of caries depends on age and is 2.07-3.04. At the same time, the percentage of healed teeth is quite low in both variable and permanent bite. When examined by an orthopedic doctor, 94.74% of children with reduced hearing were diagnosed with musculoskeletal disorders. Changes in biopotential indicators of muscles of the articulating apparatus in deaf children with existing orthodontic pathology is evidenced by their correlation. The Program "Improving the dental health of children and adolescents with hearing impairments through curative and preventive measures" is proposed, which includes a complex of diagnostic and curative measures for patients with hearing loss and orthodontic pathology, consisting of diagnostic, motivational and curative and preventive components. The diagnostic component involved establishing the type of dento-maxillary anomalies and deformations based on clinical examination, determining additional risk factors causing various bite pathologies. The motivational component provides for the establishment of contact between the doctor and the deaf child, the creation of an atmosphere of emotional comfort between the orthodontist, the deaf child and his parents, the planning of personally oriented treatment for each patient with hearing loss, the formation of positive orthodontic treatment. For this, a questionnaire was created and proposed for the first time, taking into account the specifics and features of the physical and psychological state of children with sensorineural hearing deprivation (author's certificate of Ukraine No. 69040 dated 09.12.2016). In order to increase the motivation for orthodontic treatment and to popularize orthodontics, an educational and educational video for children with hearing deprivation "A child's happy smile" was edited (author's certificate of Ukraine No. 115999 dated 19.01.2023). A questionnaire was also created for parents or guardians of hearing-impaired children (author's certificate of Ukraine No. 121956 dated 14.12.2023) and an educational video "For parents about orthodontic treatment of their special children" (author's certificate of Ukraine No. 115998 dated January 19, 2023 .).Another motivational component was the study of the emotional attitude of hearing-impaired children to important people in the family, both on a conscious and partially unconscious level, using the Atkind relationship color test, followed by its analysis. Scientific novelty of the obtained results. Added scientific data on the state of dental and orthodontic health in DHH; the prevalence of orthodontic treatment in DIC was studied, the level of need for orthodontic care was determined; new scientific data were obtained regarding the functional state of the masticatory muscles and the circular muscle of the mouth of the DHH depending on the type of ALS; to increase the motivational level of DHH and their parents/guardians for orthodontic treatment, an assessment of their level of dental and orthodontic knowledge was carried out; for the first time, a complex of curative and preventive measures was developed, proposed and implemented in the form of the Program "Improving the dental health of children and adolescents with hearing impairments through curative and preventive measures", which will increase the effectiveness of the prevention of hearing loss in DHH. Key words: malocclusion, dento-mandibular anomalies, bite disorders, deaf children, children with hearing impairment, disability, caries, level of hygiene, prevention, posture, questionnaire, electromyography, functional muscle activity, circular muscle of the mouth.
