Essay

Object of study: diabetes mellitus type 2 in combination with overweight and obesity. Purpose: Improvement of the diagnosis, prognosis and treatment of arterial vascular lesions based on the study of clinical and pathogenetic role of markers of apoptosis, fibrotic activity and humoral factors of lipocytes in patients with type 2 diabetes mellitus with overweight and obesity. Examination methods: General clinical studies, electrocardiography, chest radiography, echocardioscopy, vascular dopplerography, rheovasography, determination of blood levels of transforming growth factor β-1, caspase-3, omentine by immunoassay method, questionnaires for quality of life, statistical methods. Scientific novelty of the obtained results: New data on the pathogenetic role of fibrotic processes in arterial vessels of elastic and muscular type, features of apoptosis, functional activity of adipose tissue in patients with type 2 diabetes mellitus with overweight and obesity. The role of changes in the pathophysiological association between the activity of fibrotic processes and the intensity of apoptosis in the formation of pathological changes in the elastic properties of arterial vessels, functional activity of adipose tissue and quality of life in patients with type 2 diabetes mellitus with overweight and obesity. Based on the obtained results, a comprehensive study of fibrotic disorders and apoptosis processes and determining the role of the association of these processes in the development of hemodynamic changes and adipose tissue metabolism in patients with type 2 diabetes mellitus with overweight was further developed. The relationship between structural and functional changes of the left ventricular myocardium and the activity of apoptosis in type 2 diabetes mellitus in comorbidity with obesity has been proven. Improved prognostic and clinical-diagnostic value of omentin content as a marker of visceral adipose tissue metabolism in comorbidity of obesity and type 2 diabetes. Updated and supplemented scientific data on quality of life in people with type 2 diabetes with overweight and obesity with emphasis behavior and attitude to excess body weight during therapy (IWQOL-Lite, COEQ4 7days FPRS). The data on the clinical efficacy of dapagliflozin in the complex therapy of type 2 diabetes mellitus, in particular the effect on fibrotic processes, apoptosis, adipose tissue metabolism and quality of life in patients of this category are determined and expanded. Omentin has been shown to be a reliable predictor of the positive clinical efficacy of dapagliflozin therapy.
