According to the results of the dissertation study in 100 women of late reproductive age with impaired folate metabolism and anamnestic embryonic losses and in 40 women of the control group, scientific data on the features of the clinical course of pregnancy and childbirth in patients of late reproductive age with early loss of pregnancy in the anamnesis with disorders of folate cycle metabolism. Late reproductive age patients have been associated with a higher risk of embryonic loss with rs1801133 (677 C>T) polymorphism of MTHFR gene and no association with rs1805087 (2756 A> G) polymorphism of MTR gene has been identified. Scientific data on predictors of fetal-placental angiogenesis and reduction of vascular wall thrombotic potential in late reproductive age pregnant women with anamnestic embryonic losses have been extended, in particular, decreased serum levels of vascular-endothelial and placental growth factors (increase carriers of the minor T-allele by polymorphism rs1801133 (677 C> T) of MTHFR gene were shown. Preventive strategy of clinical management of late reproductive age women with impaired folate metabolism and history of embryonic loss was developed and adapted for practical use, taking into account the safety profile based on the introduction of prophylactic therapy using biologically active form of folate in the form of 5-methyltetrahydrofolate.
