Essay

Object of research: the course of hypertension in combination with rheumatoid arthritis on the background of complex treatment. Purpose: to improve the diagnosis and treatment of hypertension by determining the features of the clinical course, the level of metabolic disorders, endothelial function, systemic and inflammatory processes, left ventricular remodeling in patients with rheumatoid arthritis. Research methods: general clinical (patient complaints, anamnesis, physical examination), laboratory (clinical, biochemical), enzyme-linked immunosorbent assays (IL-1β, IL-6, IL-10, TNF-α), instrumental (electrocardiography, echocardiography, ultrasound examination of carotid arteries , daily blood pressure monitoring, radiography). Scientific novelty of the obtained results: scientific data on the peculiarities of hypertension combined with rheumatoid arthritis as an unfavorable comorbid pathology of high cardiovascular risk with a predominance of atherogenic dyslipidemias of type II have been supplemented; - the scientific provisions on the prevalence of frequency (in 72.6% of patients) and more severe endothelial function disorders in terms of endothelium-dependent vasodilation in patients with GC with rheumatoid arthritis, compared with the isolated course of the disease; - New scientific data on the diagnostic role of immunoinflammatory factors (IL-1β, IL-6, IL-10, TNF-α) in patients with hypertension in combination with rheumatoid arthritis and their predictor value for the prognosis of adverse GC; - probable (p <0.05) excess of proinflammatory cytokines: interleukin 1β, interleukin - 6, tumor necrosis factor - α in the group of patients with comorbidity of GC and RA compared with groups of patients with isolated hypertension and rheumatoid arthritis; - expanded understanding of the features of pathological myocardial remodeling, with a significant predominance of the frequency of concentric left ventricular hypertrophy in patients with GC with concomitant RA against patients with isolated hypertension; - presented new results regarding the nature of changes in the daily profile of blood pressure (BP) in patients with hypertension in combination with rheumatoid arthritis; - developed and scientifically substantiated the clinical effectiveness of various complex regimens of GC therapy on the background of RA with the use of perindopril at a dose of 10 mg per day or telmisartan at a dose of 160 mg per day; - significantly better effect of telmisartan on the indicators of target levels and synchronization of the circadian rhythm of blood pressure, reverse remodeling of the left ventricle, regression of endothelial dysfunction; - scientifically substantiated optimized comprehensive treatment program for patients with hypertension in combination with rheumatoid arthritis on the basis of measures to improve patients' adherence to treatment.
