Essay

The dissertation is devoted to the study of the impact of the immediate loading of dental implants on the rehabilitation processes of patients with dental defects. The obtained results of the study made it possible to introduce into clinical practice a new approach to the application of immediate masticatory load during one-stage implantation, which will not only significantly shorten the manufacturing time of permanent structures of dental prostheses, but also make it possible to assess the impact of immediate functional load on the supporting elements of prosthetic structures, which is of significant importance for prediction of treatment results. On the basis of a comparative analysis of the features of functional loads on supporting tissues, practical recommendations have been developed for the prediction and effectiveness of orthopedic treatment with the use of dental implants and immediate loading during intraoperative prosthetics, and an algorithm of standard operating procedures and a project of a patient treatment protocol at each stage of implant installation of the own system ART IMPLANT have been developed with intraoperative functional immediate load. Our goal was to investigate the specifics of the impact of immediate loading of dental implants of our own ART IMPLANT system on the processes of osseointegration dynamics and to ensure a high predicted functional and aesthetic result in the rehabilitation of patients with dentition defects. A comparative evaluation of patient satisfaction with the treatment performed showed a statistically significant difference in the evaluation of the treatment in the group of patients who used the one-stage surgical protocol of dental implant surgery compared to the group of patients who used the two-stage surgical protocol of dental implant surgery. The use of a one-stage concept and one-stage implants is an alternative to the generally accepted technique of a two-stage protocol and allows in many clinical cases to successfully avoid bone plastic surgery, which is a rather traumatic, long, expensive and insufficiently predictable procedure. One-stage (monolithic) implants are devoid of the disadvantages of two-stage (collapsible) implants, they are simpler in prosthetics, but at the same time, they require more precise positioning from the doctor during surgery. They are an inexpensive choice, especially in complex clinical cases that require simple solutions.
