Essay

The thesis presents clinical and pathomorphological data on the necessity to determine placental factors in the blood of pregnant women with isolated congenital heart disease in the fetus, ultrasound echocardiographic examinations, morphological and histochemical studies of the placenta in order to improve the efficiency of diagnosis and prognosis of CHD (Congenital Hardi Desis). The article describes the clinical, laboratory, and instrumental data of studies in 30 pregnant women with isolated fetal CHD (study group) and 60 pregnant women with a healthy fetus (control group) for the period 2020-2022 based on prospective examinations of a hospital sample. The need for a multidisciplinary approach in the management of pregnant women of different gestational ages is demonstrated. At the beginning of the study, the criteria for the selection of pregnant women for the study and control groups were defined. Analysis of the clinical data showed that isolated fetal heart defects were diagnosed in women with an uncomplicated somatic, obstetric and family history, whose average age was 28.03±5.12 years and was not statistically different from the age of women with a healthy fetus (p=0.239). The paper presents the anthropometric data of pregnant women of both groups (body weight, height, body mass index) before pregnancy and before delivery, which corresponded to the average anthropometric parameters of women in Ukraine, with no significant difference in the determined BMI in the groups (p=0.252). The multivariate logistic regression model did not reveal the influence of a woman's age and anthropometric data on the occurrence of GDM. The gestational age at the first visit to the obstetric hospital of pregnant women with isolated fetal heart disease was 28.26±8.45, which was due to the need for prenatal exclusion of other developmental defects and the decision to deliver. The gestational age of pregnant women in the study group was 38.07±0.94 weeks, which was statistically significantly different from the control group (p=0.002).
