Essay

Modern approach to the non-metastatic breast cancer treatment includes multidisciplinary approach of diagnostics and individual treatment. Surgery remains the main method of solid breast tumors treatment, especially of the ones diagnosed at early stages. Incidence rate for 100 000 population remains a tendency of every year increase and is 9,9 % in the end of 2021 year if compare to the previous one. The number of primary cases in 2021 was n=14150, when 4769 patients have died. Among women sickness and death rate structure breast cancer is at the first place among the age groups of 30–54, 55–74 and constitutes 30,1%, 21,4%. The major part of the first time revealed had stages І (19,8%) та ІІ (47,3%), that can get a definitive therapy and sustained remission. Symbiosis of connection between the breast conserving surgery and aesthetic breast shape has become the oncoplastic surgery. The oncoplastic surgery is based on the principles of safe tumor removal and plastic surgery. To classify the variety of the breast conserving surgery methods, in the literature it was accepted to use the term of “volume displacement” and “volume replacement”. Volume displacement is meaning these types of technical methods, allowing to reconstruct the breast shape by redistribute of patients own breast tissue, meanwhile volume replacement describes the reconstruction technic types, using donor flaps outside the breast. Breast shape reconstruction by volume replacement has a lot of technical difficulties and requires further improvements. Regarding the results of the overall survival and disease-free survival high levels among the patients with breast cancer, main aim to improve the quality of life and functional of unilateral upper limb ability from the operation side of treatment provided is essential. The problem of prevention and therapy of the breast cancer related lymphoedema is still actual. It is significantly important to determine and prevent the factors which may increase surgical complications risks. Dissertation aim: To research the remote functional and oncological results using the oncoplastic technics of breast shape reconstruction with the regional island perforator flaps of thoracic wall.
