Essay

The thesis is dedicated to development of prophylactic of sexual dysfunction and andrological rehabilitation of the patients after surgical treatment of benign prostatic hyperplasia (BPH) by methods of a transurethral resection (TURP) and transvesical prostatectomy. It was proved that one of complications of surgical treatment of BPH is the development of sexual dysfunction in 18,3 % of the patients. The development of a postoperative sexual dysfunction took place in groups TURP, open one moment and two moment transvesical prostatectomy in 9 %, 18 % and 32 % of cases, that allows to survey TURP by the operation of choice from positions of potency preservation. The sexual dysfunction after surgical treatment of BPH can wear both functional and organic character. The theoretically possibility of development last is proved by morphological researches in cadavers after a prostatectomy and TURP, which have confirmed possibility of a traumatising periprostatic nervous (n.n. errigentes), which provide relaxa tion on pudendal arteries and cavernous tissue. The given damages can develop at perforation of a prostatic capsule and as owing to a generalisation of inflammatory processes from prostate to the periprostatic tissue and development in it sclerotic changes The risk factors of a postoperative sexual dysfunction, including, bound with the surgical operation are defined. The algorithm of prophylactic and the andrological rehabilitation system of the patients, with base usage of a designed method VTD- electrical therapy is offered.
