Kurochka I. Surgical treatment of "gigantic" duodenal ulcers

Dissertation includes results of complex clinical, instrumental and immunological observance of 73 patients with gigantic ulcers (GU) of duodenum. Duodenal GU are one of variants of peptic ulcers and occur in 9,6% of cases. Nontypical and heavy clinical current are among their pecu-liarities. Duodenal helcomenia and perforation are established in 43,8% of cases. GU are characterized by secondary immune deficiency on rather supressoric variant is formed, increase of autoimmune and immunocomplex reactions. Partial gastro-duo-denal-ectomy with extraction of the ulcer is the most radical method of treatment. For all that when ulcer is localized in postbulbaric section of duodenum, the necessity for implantation of bile papilla and formation of cholecystostoma appears. Early and late results of operation are favourable in case of correct method of treatment in majority of patients with GU.
