The work is devoted to the problems of differentiated tactics of organ-preserving operations in pancreas dysfunction in patients with complicated duodenal ulcer. Main directions of secondary pancreatitis development in complicated duodenal ulcer were studied using pathoanatomical material. In pre-operative period in patients with duodenal ulcer, structural functional disturbances of the pancreas were noted. Clinically they manifested as acute or chronic pancreatitis. In the controls, structural functional changes of the pancreas after organ-preserving operations with the use of different types of vagotomy were studied. The revealed problems were analyzed, the ways to solve them were determined. In the study group, differentiated tactics of organ-preserving operations in complicated duodenal ulcer and accompanying acute and chronic pancreatitis was introduced. New surgical techniques which improved the results of the surgery were used. A therapeutic algorithm in patients with complicated duodenal ulcer with secondary pancreatitis was worked out. Comparative evaluation of the efficacy of the existing and suggested techniques by comparison of treatment results in the study group and in the controls was done.
