Essay

Object of the study: women with progressing tube pregnancy.Purpose of the study: Working out, theoretical validation, testing and introduction into practice of a new technique of laparoscopic linear salpingotomy followed by restoration of the uterine wall integrity using bipolar electrocoagulation of the incision margins at treatment of progressing tube pregnancy for preservation of the uterine tubes with completely preserved and restored morphofunctional properties. Methods of investigation: experimental, clinico-instrumental, radioimmune, immunoenzyme, clinico-laboratory, morphological and statistical methods of investigation were used. Practical significance: The research proved high efficacy of the worked out technique of laparoscopic salpingotomy in treatment of the patients with progressing tube pregnancy. Clinical efficacy of the technique on preserving normal anatomical morphological structure of the uterine tube was 88.2%, efficacy of reproductive function restoration was 51.5%. This method reduces the number of complications and relapses. Theoretical concepts and practical recommendations are used in teaching process at Obstetrics and Gynecology Departments No. 1 and 2 (Kharkiv State Medical University), Obstetrics and Gynecology Department of Kharkiv Medical Academy for Post-graduate Training.Novelty: For the first time, a new improved technique of laparoscopic linear salpingotomy which allows to optimize and change the generally accepted technique of laparoscopic salpingotomy was worked out. The new method improved salpingotomy technique and allowed to preserve anatomical functional characteristics of the uterine tubes, which is absent in Semm's technique of laparoscopic salpingotomy. A new is the approach to restoration of the integrity of the wall of the uterine tube after salpingotomy which is achieved by bipolar electrocoagulation of the incision margins with 20 W electric current, which does not produce obliteration in the lumen of the operated tube (Patent of Ukraine No. 35240A dated 15.03.2001).Introduction: main concepts of the work were introduced into the practical work of City Clinical Maternity Hospitals No. 1, 5, 7 (Kharkiv), Regional Clinical Hospital, Railway Hospital (Kharkiv).Field of application: Medicine, obstetrics and gynecology.
