Vasyl'yev O. The foundations of selection of means and capacity in operative treatment of patients with acute cholecystitis of the cost of operative risk

The research is based on results of inspection and surgical treatment of 747 patients. The Discriminant Function analysis of the factors of operational risk has allowed to reveal five groups of the factors of operational risk and to develop system of a ball estimation of a degree of operational risk allowing to relate the researched patient to group of high or low operational risk. The efficiency and expediency of application at the patients of group of high operational risk at the first stage of surgical treatment palliative of the operation directed on decompress and sanation of inflamed gallbladder - is proved that defines rescue of life to the patient. By operation of a choice at such patients was percutaneus cholecystostomy. By such patient by the second stage is carried out cholecystectomy. The application of advanced surgical tactics has allowed to lower number early of postoperative complications from 6,9 % to 2,1 %, and mortality from 2,2 % up to 0,8 %.
