Lunin A. Modern possibilities of surgical treatment of giant ventral hernias with usage criteria's of prognosis and prophylaxis of bronco-pulmonary complications.

The work is devoted to actual problem of surgery - surgical treatment of giant hernias at patients with estimation danger of appearance bronco-pulmonary complications in postoperative period. Clinical part worked out on 134 patients with giant postoperative, spontaneous epigastric, paraumbilical and inguinal hernias. In investigation was use computer spirography, integral reography, zonal reopulmonography. Also was studied pathomorphological changes in abdominal wall. On the basis of morphological, clinical, laboratory results and experimental investigations we discovered the optimum methods of pre-operational prepare and operational technique from point of view increasing of intraabdominal pressure. We established the role of surfactant of lungs in preventing of development bronco-pulmonary complications in postoperative period. On the basis of this was proposed method of 2-step-operation in patient with pathology of cardiovascular and breath systems. Experimental part of work included 61 white rats.Was used biochemical, biophysical investigations. The discovered methods of preoperative preparation and surgical techniques decrease the recurrent in 8,3% and postoperative bronco-pulmonary complications in 25%..
