Object of the study: Benign ovarian tumors, tumor-like formations of the ovaries and diseases, which mimic them, healthy ovarian tissue and that obtained during surgery. Purpose of the study: to improve the efficacy of diagnosis of malignant tumors and tumor-like diseases of ovaries and the diseases mimicking ovarian tumors by working out optimum methodological approaches to performing magnetic resonance imaging in voluminous ovarian neoplasms. Methods of the study: examination of the objective state of the patients using general clinical and instrumental studies, ultrasonography and magnetic resonance imaging of the small pelvis, CA-125 determining in the blood, histological, biochemical studies, statistical methods for evaluation of the significance of the obtained findings. Practical significance of the obtained results: consists in working out and introduction into practice MRI of small pelvis, which allows to do their anatomical topographical evaluation. The reference values used for US study wereshown to be suitable for qualitative assessment of MR images. The recommendations on reasonability of MRI application in tumors and tumor-like diseases of the ovaries as well as the diseases, which mimic them, were given. The role of MRI as a diagnostic technique used in gynecology was defined. The use of qualitative MR parameters specific for each disease increases accuracy of the diagnosis, acceleartes the examination and allows to refuse from the use of invasive techniques in a number of cases. Novelty of the obtained results: For the first time, large clinical material was used to study diagnostic significance of MRI in patients with voluminous ovarian neoplasms. MRI was shown to be a promising noninvasive method of examination which allows to obtain valuable information about the state of internal reproductive organs. For the first time, complete evaluation of MR images of ovaries in healthy women and in gynecological patients was done. It included qualitative characteristics spin-spin relaxation time,proton density, signal intensity) which allow to assess objectively the state of internal reproductive organs. High diagnostic value of the technique was shown, diagnostic and differential dagnostic criteria of gynecological pathology were formulated. MRI was determined to reveal zonal anatomy of the reproductive organs, pathological states associated with their disorders, differentiate benign tumors and tumor-like ovarian diseases as well as to suggest the morphology of the ovarian structure. Diagnostic value of MRI was established to be high in diagnosis of cysts and cystomas of the ovaries. Introduction: The obtained results allowed to work out an algorithm for examination of the women with suspected ovarian tumors which is used in the work of gynecology departments of City Clinical Maternity Hospital with Neonatology Department (Kharkiv), Clinical Specialized Maternity Hospital No. 5 (Kharkiv), Clinical Maternity Hospital No. 7 (Kharkiv), Kharkiv Regional Clinical Hospital.
