Pepenin S. Clinic-morphological of feature of chronic pyelonephritis of a unique kidney (UK) (clinico-experimental research)

The revealing of factors promoting development of a chronic pyelonephritis of unique kidneys (HPUK), to studying of its patogenesis and clinical current by comparison of morphological attributes of illness with clinical, laboratory, biochemical, immunological, ray, ultrasonografy. 130 patients HPUK who have transferred nefrectomy 5-10 years ago are surveyed. For an establishment of features of development compensate-adaptive reactions in UK the experimental research on 89 white mice of Vistar line by which it has been executed left-hand or right-hand nefrectomy is lead, and also the condition of the YGA in UK which has remained after nefrectomy with 26 patients by intraoperational or through-skin byopsy is investigated. It is established, that compensate-adaptive reactions of UK have the phase character dependent on amplification of a blood-groove and ultrasructure changes in cells of the YGA and kidney canaliculy that results in amplification of activity of cells of wind canaliculy and to formation new peritubular limfocapilars. Full reorganization comes to an end in 6 months. Inflammatory process in UK is accompanied by infringement activity enzymes in whey of blood and urine, progressive decrease in nonspecific immune resistency of an organism, infringements in POL-system. The algorithm of diagnostic values of various symptoms and trouble-shooting tests for diagnostic HPUK is offered.
