Gerasimenko V. Optimization of surgical treatment of diffuse toxic goitre on the basis of morphodiagnostic criteria

The work is devoted to the development and introduction in medical practice of optimized tactics for the surgical treatment of diffuse toxic goitre on all its stages. As a result of morphological researches two main morphological variants of diffuse toxic goitre with the different histological structure, histochemical and imunohistochemical parameters were determined. It is parenchymatous variants A and follicular variants B. The carried out researches allowed to differentiate the approach to the thyroid resection volume. More sparing resection is reasonable for the morphological variant B because of higher risk of hypothyroidism in the postoperative term. The operation at morphological variant A should be more radical because of an opportunity of postoperative relapse of diffuse toxic goitre. Application of the optimized surgical method provides the long-term normalization of the patients’ clinical condition, the positive processes in the immune status, the normalization of thyroid hormones levels, that reduces the frequency of the postoperative hypothyroidism and relapse of disease.
