Dissertation is devoted to the problem of the increasing of the effectiveness of the nephron's structural elements damage diagnosing and improving of the treatment of the patients with the closed kidney injury and after extra corporal shock-wave nephrolithotripsy. Diagnostic value of the investigation of neutral a-glycosidase and N-acetyl b-D- gexosaminidase activity in urine for the diagnostics of the damage level of nephron's structural elements at kidney injury has been established for the first time. It has also been established that extra corporal shock-wave nephrolithotripsy leads to increasing in urine marking ferments of the nephron's structural elements damage, neutral a-glycosidase and N-acetyl b-D- gexosaminidase, which stay increased up to the 30th day, that corresponds to kidney parenchyma at closed kidney injury in everyday life of the light level. For the first time it has been developed the experimental model of the dosed closed kidney injury for the investigation of the morphological changes dynamics, observed at kidney tissue damage during the injury in everyday life and after distant lithotripsy. It has been investigated the dynamics of the morphological changes in kidney tissue during the experimental closed kidney injury which can be the model of its damage during distant nephrolithotripsy. Taking into account the character of the found morphological damages at experimental animals with the aim of their correction and prophylactics of early and late complications, the original complex approach to the closed kidney injury treatment has been developed and its effectiveness has been proved. The expediency and the proper terms of the usage of the system Vobenzyme enzyme therapy together with hyperbaric oxygenation for the patients with the closed kidney injury have been grounded. For the first time the expediency of the investigation of neutral a-glycosidase and N-acetyl b-D- gexosaminidase activity in the urine for the estimation of the effectiveness of treatment of the patients withthe closed kidney injury of the light level has been proved.
