Gavrikov A. Subtotal and total colectomy at tumoral ileus of the left half of the colon.

Object of research: tumoral ileus of the left half of the colon. The purpose of research: improvement of results of surgical treatment jf tumoral ileus of the left half of the colon by introduction of subtotal and total cjolektomy with primary ileosygmoid and ileorectal anastomosis. Methods of research: clinical, laboratory, radiological and ultrasonic method, the retrospective analysis of case records with questioning of patients and processing of results by methods of variational statistics. Practical value of the received results: Offered surgical tactics at treatment of patients with tumoral ileus of the left half of the colon has allowed to reduce frequency of postoperative complications, number of relapses of a cancer of a thick gut and to improve quality of patients life. Essentially new drainage of the abdominal cavity based on application of a helicoid drainage tube, capable to catch intrabelly separated on the big extent with an opportunity of restoration of drainage function is developed andconfirmed with the patent of Ukraine in case of reduction of her efficiency. Scientific novelty of the obtained results: As a result of the carried out researches efficiency in comparison with traditional operations is shown and the expediency expanded primary-regenerative radical operations is proved at treatment tumoral ileus of the left half of the colon, it was given indications, contra-indications to their performance. Formation primary анастомозов by a single-row seam by nychrom wire and mycrojeunostomy for postoperative decompression and early postoperative enteral feeding is proved at the expanded operations at patients with colon obstraction. The new kind of a drainage of abdominal cavity with application of the helicoid tube providing adequate drenage is offered. The circuit of an estimation of quality of a life of patients is offered and the analysis of the late results of treatment of the patients who have transferred operations concerning tumoral ileus of the left half of the colon with primaryanastomosis, including questions of medical, labour and social rehabilitation is executed.
