Object - 214 patients with arterial hypertension; aim - to evaluate the diagnostick and treating approaches in conducting of patients with perioperative arterial hypertension on the basis of studying of clinic and pathogenetic peculiarities of hypertensive syndrome in perioperative period. Methods used in the investigation: clinical, instrumental, biochemical, questioning. New trends: perioperative hypertension - is independent for first time appeared form of hypertension and is a factor risk of development hypertension disease. Their timming is dependent on several factors, including the multifactorial, genetics, neiro-regulation, urgency of surgery, patient-specific risk factor, and surgery-specific consideration. The scientific work presents the system analysis of the functional conditions of patients who was under went to surgery. This study was designed to present more completely the biological entity of the processes in organism during and after surgery. It was revealed that blood group and Rh factor play a significant role in development of AH. The most unfavourable combination from this point is IV group Rh(-), but the same Rh(-) unfavourable too. It was made for prediction of perioperative hypertensive reaction and choice of an optimal therapy. The clinical and diagnostics algorithm was developed - carvedilol monoterapy was superior to metoprolol in terms of hypotensive and clinical effects, restore physiology reaction neirohymoral regulation due to its combined blocker of the alfa1and beta-adrenergic receptors among patients with preoperative and essential hypertension, and metoprorol - with postoperative hypertension.The results of investigation are introduced in studying and practical medicine; sphere - medicine.
