Dissertation devoted to determination of minimally acceptable level of hematocrit, which provides adequate relationship of systemic oxygen transportation and oxygen demands of the organism in cardiac surgical patients in early postperfusion and early postoperative periods. Based on the results of examination of 207 patients, was researched relationship of systemic homodynamic, functional condition of the heart and oxygen transport function of the blood from hematocrit. Was determined and confirmed minimally acceptable and optimal level of hematocrit and level of hemodilution in cardiac surgical patients, when signs of hemic hypoxia did not appear. Were developed and introduced in practice algorithms of intensive care in cardiac surgical operations with limited use of donor blood and its components, improved tactic of anesthesia in operations performed after "blood free" method and operations, associated with massive bleeding, that gave possibility to decrease use of donor erythrocytes in 2.4 times, andto decrease number of complications, associated with impairment of gas transporting function of the blood from 22.2% to 10.6%.
