Dubinin M. Effectiveness increase of urinary derivation after cystectomy by the way of dolichosigmoid application for shaping the orthotopic urinary bladder

The dissertation is devoted to the problem of the effectiveness increase of treatment of patients with invasive forms of cancer of urinary bladder by the way of perfection of surgical technology of neocyst orthotopic reconstruction after cystectomy on the account of the additional loop of sigmoid colon. The modification is worked out and the surgical technology of the urinary bladder orthotopic reconstruction out of the additional loop of the sigmoid colon (dolihosigma) is improved - there are 42 observation combining the advantages of detubularized reservoirs of the low pressure with the antireflux defence [patent of Ukraine №51360A]. The complex dynamic research of the microbiocynos of orthotopic urinary bladder formed out of the additional loop of the sigmoid colon is made; quality and quantity characteristics of the biotype transformation and potentially dangerous periods for the development of pyoinflammatory complications are defined. Prognosis aspects of the patients survival after the radical cystectomy, depending on the initial phase of the malignant process and the stage of histopatalogical differentiation of the tumor have been studied. The comparative analysis of the nearest and remote outcome of the modified dolichosigmoneocystoplastic, the dynamics of the relaxing and morphofunctional adaptation as well as the level of life of the patients has been carried out.
