Object of the study: Discoordinated labor activity. Purpose of the study: To optimize the algorithm of labor management, improve the efficacy of drug correction of discoordinated labor activity for reduction of intranatal complications and frequency of surgical interventions. Methods of the study: Clinical, biochemical, immunoenzyme, microbiological, histomorphological, instrumental (ultrasonography, hysterography, Doppler ultrasound, car-diotocography, pulsometry) mathematical statistical. Practical significance: The author suggests an algorithm of labor management and drug correction of DCLA including hysterography, Doppler ultrasound, CTG, biophysical fetus profile, pulsoximetry as well as determining AC, CE, A, cGMP, O and PGF2?. A technique of combined spasmolytic correction of DCLA at initial stages of its development which includes Spasmobrue - 10.0 mg (half bolus dose) + NO-Spa - 40.0 mg allowing to nor-malize the uterus activity, improve hemodynamics and the state of the fetus, complete the labor per vias naturalis in 8.9%, reduce the incidence of side-effects at simultaneous potentiation of myorelaxation, reduction of requirements in tocolysis with ?2-adrenomimetics up to 11.9%, reduce the number of injuries as well as the percentage of cesarean sections was suggested and clinically tested. The findings of the study of latent persisting urethro-vesical infection in women with DCLA allowed distinguishing a risk group of danger of labor activity disturbances. Novelty of the obtained results: For the first time, it was proven that higher level (36%) of latent nonspecific infection of the genital tract and pathogenic potential of persisting microflora are present in patients with DCLA. It was established that a leading initiating moment of DCLA development is over-excitation and dysbalance of sympathico-adrenal and cholinergic nervous system, which substantiates prevailing influence of maternal hormones which de-termine uterotonic action (PG2? ,О, S). Complex study showed that the following play an important role in realization of clinical effect of combined spasmolytic treatment: a) neurovegetative blockade of the peripheral anti-cholinergic me-diator mechanisms which reduces AC content and secondary mes-senger cGMP at simultaneous increase in antitoxin-esterase activity and cAMP level, b) influence of the CNS in the central mechanism, anti-stress sedative effect, c) potentiating total myorelaxant effect, d) relaxation accompanied by reduction of the power of labor, normalization of its frequency, configuration and gradient as well as significant (possibly secondary) reduction of A, S and uterotonic substances (O and PG2a) concentration, which allowed to sub-stantiate new approaches to treatment of this complication. Introduction: The worked out practical recommendations were introduced to the work of maternity department of Kharkiv Re-gional Clinical Hospital, Ukrainian Research Medical Center for Obstetrics, Gynecology and Reproductology (MH of Ukraine), maternity hospital No. 1 (Kharkiv), maternity department of gen-eral hospital No. 17, maternity department of Lozavaya hospital. The findings of the work are used in the teaching process at obstet-rics and gynecology department of Kharkiv State Medical Univer-sity and Kharkiv Medical Academy for Post-graduate Training.
