Essay

The dissertation presents high diagnostic value of Dobutamine stress-echocardiography (DSE) for the detection of ischemic heart disease and hibernating myocardium, reveals correlation between coronary artery (CA) damage and development of ischemia during the test. DSE is more effective method for the detection of ischemic heart disease, then veloergometry, especially for the patients with arterial hypertension and hypertrophy. Worsening of contraction in one segment of left ventricle is enough diagnostic criterion for the positive test, angina pectoris with ischemic ECG changes increase its specificity. Peak summary wall motion index more then 1,5 and development of constriction disturbances in more then 6 segments of left ventricle increase probability of severe summary damage of CA. High positive and negative predictive value of DSE for the detection of hibernating myocardium was shown.
