Object of the research: oxidative modification of proteins, nucleic acids, phospholipids and bioenergetics and metabolic provision of pro-, antioxidant protection of patients with psoriasis. Purpose of the research: increase of efficiency of complex treatment of patients with psoriasis provided provision of systemic antioxidant correction of violations of bioenergetic and redox processes at the level of lipid peroxidation, oxidative modification of proteins and nucleic acids. Methods of research: a complex of clinical (anamnestic examination according to a special program, clinical-topographic, general clinical methods and monitoring at the stages of treatment), morphological (dynamic control of efflorescence), biochemical and biophysical methods (oxidative modification of proteins and nucleic acids, aerobic and anaerobic glycolysis and bioenergetics of the cell, as well as lipid peroxidation and enzymatic chain of antioxidant protection); a number of clinical-statistical methods is used, particularlythose, which are based on parametric and nonparametric statistics, probabilistic distribution of the signs and estimation of reliability of the results. Practical significance of the obtained findings: a systemic approach to improvement of diagnosis and treatment is provided, based on taking into account pathogenic mechanisms and clinical, functional, metabolic and morphological efficacy of antioxidant preparations in the system of a complex treatment. A system of eruption registration according to coordinate and functional dermatomas is scientifically substantiated and used, application of which in combination with an integral index of severity and prevalence of psoriasis improves the system of methods of dispanserisation of patients with psoriasis; a system of morphochronography of psoriasis eruptions can be used when estimating the efficacy of a complex treatment with application of antioxidant preparations in the system of clinical monitoring of patients. It is proved that the use of antioxidants when treating patients with psoriasis is characterized by a systemic metabolic effect, which in case of using the preparation “Thiotriazoline” allows to provide the highest clinical-methabolic, clinical-morphological efficacy. Scientific novelty: consists in the enlargement of ideas concerning the role and mechanisms of realization of an oxidative stress in system dermatoses, in particular, during psoriasis. It is proved for the first time that the processes of free-radical oxidation are revealed simultaneously and characterised by the definite order of metabolic changes at the level of lipid peroxidation, oxidative modification of proteins with deepening of their destruction and nucleic acids with characteristic change of bioenergetic processes at the level of aerobic and anaerobic mechanisms of glycolysis and bioenergetics of the cell. Common factors for different clinical forms of psoriasis are determined for the first time, promoting manifestation and formation of its exacerbations, as well as topography of primary eruptions with classification by coordinate and functional dermatomas and regions. Presence of an isolated influence of the age and sex on the intensity of dismetabolic violations of antioxidant protection of patiensof violated antioxidant protection. For the first time, by the data of clinical monitoring of patients, presence of violations of bioenergetic and metabolic provision of antioxidant protection characteristic for the stationary and progressive stages of psoriasis is proved, they are important for estimation of state of the free-radical oxidation at the stages of complex treatment, as well as the course of morphologic changes of psoriatic eruptions under the influence of improved treatment. It is proved that antioxidant therapy of patients with psoriasis is characterized by a system metabolic influence with its correspondant distribution at the levels of correction of oxidative stress, and antioxidant preparations – by clinical-metabolic targets. Degree of introduction: The data concerning increase of efficacy of complex treatment of patients with psoriasis by means of a system antioxidant correction of bioenergy and free-radical processes and developed author’s systems are applied in clinical studies, as well as in educational process and in the system of post-graduate education of specialists of O. O. Bogomolets National Medical University, the Kharkiv Medical Academy of Post-graduate Education of Ministry of Health of Ukraine, Institute of Dermatology and Venerology of AMS of Ukraine, the Kharkiv State Medical University, National Pharmaceutical Academy of MH of Ukraine. The method of treatment and system of clinical monitoring processed by the results of the study, are introduced into medical-diagnostic process in Kharkiv and Cherkassy regional, Romny and Kharkiv town dermatovenerologic dispensaries, in policlinic department and the Hospital of the Institute of Dermatology and Venerology of Academy of Medical Science of Ukraine.
