Object of the study: Acute adhesive ileus. Purpose of the study: To improve the results of treatment of pa-tients with AAI with the consideration of diagnosis, prognosis and treatment of syndrome of abdominal compression. Methods of the investigation: Biochemical, immunological, morphological, x-ray, instrumental (echosonography, spirometry, transvesical measurement of the intraabdominal pressure, in-traintestinal and rectal manometry, umbilical vein catheterization), statistical methods. Practical significance of the obtained results: Revealing a dis-tinct correlation of IAP, the pressure in the duodenum and rectum allows to assess intraabdominal hypertension based on the pa-rameters of intestinal manometry. The suggested method of prog-nosis and treatment of compartment syndrome allowed to reduce the number of complications of AII and death rate. Novelty: Based on the study of morphological and functional dis-orders in the most sensitive to intraabdominal hypertension organs (lungs, liver, kidneys) the role of abdominal compression syn-drome in development of syndrome of systemic inflammatory re-sponse and polyorgan failure was proven. Dependence between intraabdominal pressure, pressure in the duodenum and rectum was determined based on the calculated correlation. Taking into consideration the findings of intraabdominal pressure and parameters of systemic inflammatory process cytokines, namely IL-8, a method of prognosis of polyorgan insufficiency in patients with acute intestinal ileus was worked out. A complex of measures on treatment of abdominal compression was worked out with the consideration of leading pathogenetic aspects of its development and its consequences.
