Podlyesnyj V. The prophylaxis of stump duodenum inability during surgical treatment of complicated low- set duodenal ulcer

Object of stady: complicated duodenal ulcer decease with low-set ulcer. Purpose of the stady: to improve the results of surgical treatment of sick men with complicated low-set duodenal ulcer by developing and applying a number of measures on duodenum stump inability prophylaxis.Methods of the study: general clinical, laboratorial, endoscopic, rontgenological, morphological, statistical.Practical significannce: the criteria of differentiational approach to solving the problem of duodenum stump inability during surgical treatment of complicated low-set ulcers have been developed. The developed systematization of risk factors of duodenum stump inability origin allows to determine the basic trends, a number of preventive measures and individualize them in each particular case. Application of the method of plastic closing of open duodenal stump lessens the risk of stump inability. Clarifying the intra-operative criteria of chronic abnormalities of duodenal permeability allowed to ground indications for duodenum stump decompression with the help of drainage process as a preventive component concerning stitches instability. Practical application of the offered complex approach to duodenum stump inability prophylaxis during surgical treatment of complicated low-set duodenal ulcer allowed to improve the operative treatment results of sick men of the given category. Novelty of the obtained results: solving set problems the author has studied clinic peculiarities of complicated low-set duodenal ulcers. For the first time the systematization of the factors that favor to duodenal ulcer inability has been done and the algorithm of its complex prophylaxis has been developed. The importance, volume and stages of preventive measures have been proved. The method of plastic closing of open duodenal stump during surgical treatment of complicated low-set ulcer has been developed. First the possibility of intra-operative criteria application in order to diagnose chronic abnormalities of duodenal permeability has been studied. On the base of these criteria the indications for duodenum stump decompression have been determined. The efficiency of duodenum stump decompression has been proved by means of internal drainage procedure along with the method of its closing in the complex of stitches instability prophylaxis in the surgery of complicated low-set duodenal ulcers.
