Object of the study: hyperfunction of the thyroid gland (TG) in pregnancy. Purpose of the study: to improve the course and consequences of pregnancy in women having hyperfunction of TG on the basis of the detection of pathogenetic mechanisms in the development of obstetric and perinatal complications, to work out and introduce a new medical-prophylactic complex. Methods of the investigation: clinical-statistical; supersonic investigation, use of immunoenzyme method for detecting the concentration of hormones (estradiol, estriol, progesterone, chorionic gonadotrophin, placental lactogen, thyrotropic hormone, triiodothyronine, tetraiodothyronine), morphological study of the placenta. Theoretical and practical results: as a result of the conducted studies, a complex of clinical, laboratory, instrumental methods of investigation was suggested, thereby making it possible to assess disturbances in the main links of the pathogenetic chain in hyperfunction of TG within the early terms of pregnancy. Expediencyof including Mildronat drug into combined therapy for normalizing the endocrine function of TG and foetoplacental complex (FPC), increasing the activity of biooxidants, improving vascularization and metabolism of the shaggy chorion of the placenta was proved. The devised therapy makes it possible to reduce the rate and severity of complications in the course of pregnancy and improve the state of the newborns, it can be used at both inpatient conditions and female dispensary. Novelty: as a result of the research, new data were obtained about pathogenetic mechanisms of the development of complications in the course of pregnancy and labour in women with the hyperfunction of TG. For the first time, it was revealed that hyperfunction of TG in pregnancy significantly increases the rate of chronic placental insufficiency owing to disturbances in the hormone-producing function of FPC. For the first time, it was proved that chronic placental insufficiency resulted from a distur-bance in the maturation of the shaggy chorion against a back-ground of its worsened vascularization, intensification of involutional and sclerotic processes. Activation of the process of lipid peroxidation and a deficit in the activity of the first link of the antioxidant system of the organism’s protection were revealed. Modern approaches to early diagnosis, treatment and prophylaxis of perinatal complications in women with the hyperfunction of TG were improved. Degree of introduction: the obtained results of the research were introduced into the practical work of obstetric departments of the Kharkiv Regional Hospital, the Department of Pregnancy Pathology of Maternity Home No. 1 of the City of Kharkiv, the Department of Pregnancy Pathology of the City Maternity Home with a neonatal inpatient department, the obstetric department of the Ukrainian Research-Practical Centre of Obstetrics, Gynaecology and Reproductology of the Ministry of Health of Ukraine, the Maternity Department of the Sakhnovshchina Central District Hospital, the Maternity Department of the Kolomak Central District Hospital. Theoretical propositions and practical recommendations of the thesis are used in the pedagogical process at Department of Obstetrics and Gynaecology No. 2 of Kharkiv State Medical University, departments of Kharkiv Medical Academy of Postgraduate Education.
