Essay

Object of the study: colic haemorrhages (CH) of the tumour genesis (TG). Purpose of the study: improvement of the results of treating pa-tients with CH of TG by working out the optimum diagnostic-medical tactics and new methods of surgical treatment. Methods of the investigation: general clinical, clinical-laboratory, biochemical, morphological, instrumental (rectosig-moscopy, fibrinocolonoscopy, irigoscopy, ultrasonography), his-tological examinations, statistical methods. Theoretical and practical results: on the basis of the haemor-rhage course study, the notions of the stable and unstable haemo-stasis in patients with CH of TG were substantiated. On the basis of the findings, obtained when examining the clinical-anamnestic and laboratory data in patients with CH, the clinical picture of CH was studied and an algorithm was suggested, the latter made it possible to significantly improve the quality and reduce the terms of diagnosing CH. A stage-by-stage tactics was suggested for treating CH of TG, depending upon the variant of CH. A low-invasive haemostasis method was devised and used on patients with a rectal haemorrhage, thereby making it possible to arrest CH in cases of the conservative therapy failure. Novelty: the author conducted a complex comparative study of clinical manifestations of CH of TG and peculiarities in the course of CH with regard for anamnestic and clinical-laboratory data and accompanying complications. The most important, from the viewpoint of diagnosing, homeostasis disturbances, abilities and characteristic signs, revealed during additional methods of examination, were found out. For the first time, the abilities and indications for applying low-invasive methods of haemostasis in CH of TG and their efficacy were studied. A stent was worked out for arresting haemorrhages from the rectum. Different methods of CH treatment were studied and comparatively characterized. On the basis of the obtained data, a diagnostic algorithm was written and the stage-by-stage surgical tactics forCH of TG was individualized. In a comparative aspect, the clinical-diagnostic dynamics when using the traditional and author’s own methods and the noninvasive dynamic control over the treatment efficacy were studied. Degree of introduction: results of the researches were introduced into the work of surgical departments of the Institute of General and Urgent Surgery (Academy of Medical Sciences of Ukraine), Meshchaninov Kharkiv Clinical Hospital of First Aid. Sphere of application: medicine, surgery.
