Essay

The dissertation is dedicated to the increase of surgical treatment efficiency in patients with liver metastatic cancer. 91 patients with metastatic cancer of the liver (75 % of colorectal cancer metastases) in this work were divided into two groups. Comparative group performed by 49 patients in which the surgical resection of the liver was done. In the main group in 42 patients was used the radiofrequency ablation. Also in investigation were include the patients with synchronous metastases (55 patients, 60,4 %) and methachronous metastases (36 patients, 39,6 %). The diagnosis of solitary metastases was diagnosticate in 42 patients (46,2 %), the multiply metastases in 49 patients (53,8 %). The application of electrodes was done by two methods: percutaneous introduction under the ultrasound navigation (in 30 patients were destroyed 81 metastases) and during laparotomy (in 12 patients were destroyed 31 metastases). The results of the treatment were learned in 80 (87,9 %) patients (in 41 after liver resection, in 39 after radiofrequency ablation). The censored survival in terms of 36 months was learned. 1-year survival in both groups consist of 81,2 %, 2-year survival - 39,1 %, 3-year censored survival - 5,6 % for group of radiofrequency ablation. The median of survival for both group consist of 20 months. The improvement of results of the surgical treatment in patients with metastatic liver cancer must be look like a complex approach. The methods of surgical resection and radiofrequency ablation are the important components of the anticancer treatment.
