Dissertation is devoted to the ground, research and introduction in clinical practice of optimum variants of the anesthesia of the first medical and skilled aid to the patients with the trauma of the hand different degree of severity. It is experimental represented of the original methods of modeling of nociceptive pain, catheterization of brachial plexus, verification of regional anesthesia; efficiency of application of brachial plexus block in comparison to application of opiate's, presents the objective criterion of nociceptive pain in the hand. In clinical part of dissertation investigated the homoeostasis in patients with the trauma of hand of a different degree of severity at use of brachial plexus block in the traumathology room, urgent and planned operating-room. On the basis of epidemiology researches and study of clinicals, biochemicals, bioelectrics, psichophisiologics indexes in patients with the trauma of hand proved, that the type of regional anesthesia of brachial plexus is determined by degree of trauma severity of hand. For prognosis of the adaptation possibilities of organism and subjective feelings of patients the effectively use next informing indexes: linear equalizations of regression, partcial pressure of carbon dioxide in a blood, level glucose in a blood, amount trombocites, amount leucocytes, level of haemoglobin, level of general billirubin, age of patients, level of lactat in a blood.
