Object of the study: arterial hypertension (AH). Purpose of the research: improvement in the efficacy of treating AH patients on the basis of the study of peculiarities in their immune status, function of the endothelium, vascular-thrombocytic and plasmatic haemostasis. Methods of the research: general clinical, laboratory, bio-chemical, immunological, instrumental, and statistical ones. Theoretical and practical results: the prognostic significance of a complex study of the indices of vascular-thrombocytic and plasmatic haemostasis, the levels of serum interleukin-8 (IL-8), endothelin-1 and selectins E and P, the immune status in patients with different degrees of the severity of AH was established. Correlations between the type of the haemodynamic response to a dosed physical load and the degree of thrombocytic activation, immune status and the content of intercellular mediators in different degrees of AH were revealed. It was proved that, when comparing types of haemodynamic response to a physical load with the assessment of thrombocytic activity, the state of the endothelium and immune status, it was possible to prognosticate the further development of AH, thereby enabling doctors to individualize the treatment of these patients. A clinical ap-probation of a combined method of treatment was conducted with resultant data, which demonstrate a directed character of the effect and efficacy of using Advocard for the treatment of this kind of patients. Novelty: for the first time, a complex study of the indices of the thrombocytic and plasmatic haemostasis, the levels of IL-8, endothelin-1 and selectins E and P, the immune status in patients with different degrees of the severity of AH was made. A previously unknown relation between the physical load and the state of the thrombocytic and fibrinolytic links of haemostasis in patients with AH grades II and III was demonstrated. The interaction between the level of serum cytokines with the degree of adhesion and aggregation of thrombocytes at the state of AH progression was analysed. A new way was developed for prognosticating the further development of AH by comparing types of the haemodynamic response to a physical load with the assessment of the activity of thrombocytes, the state of endo-thelium and immune status in patients with AH grades II and III, thereby making it possible to individualize the treatment of these patients. The notion about the state of the functional activity of the endothelium, thrombocytes, fibrinolytic and anticoagulant links of the blood coagulation system in the conditions of AH appearance was made more profound. A necessity of a complex study of the immune status, the activity of thrombocytes, the function of the endothelium depending upon the severity of AH, the degree of the endothelium lesion, an affection of the immune status and function of thrombocytes was grounded. The efficacy of modern modes of AH correction by an assessment of the initial level of the own activity of thrombocytes, endothelium and immune status was established. The inclusion of Advocard into complex treatment of AH patients aimed at secondary prophylaxis of thrombogenic disorders was pathogenetically grounded.
