The dissertation is dedicated to the solution of scientific and practical problem of increasing the effectiveness of rehabilitation of reproductive function for women suffering from tubal infertility by optimisation of diagnostics, treatment and prevention of tubal infertility taking into account anatomic and functional changes of uterine tubes and level of adhesion process intensity. Integrated studies have shown that objective defining of functional state of uterine tubes for women suffering from tubal infertility needs evaluation of not only anatomic uterine tubes patency and of adhesion process intensity via laparoscopy, but also of the state of the walls of uterine tubes via morphologic and immunohistochemical researches. A new method of prediction of reproductive termination for women suffering from tubal infertility is suggested. This method allows to identify reproductive prognosis with to within 86,3% and to choose the patients, for whom treatment with the usage of laparoscopy surgeries will be effective. A new method of treatment and prevention of tubal infertility has been developed based on carrying out laparoscopy reconstructive plastic surgeries with additional intraoperative prescription of antiadhesion gel and further definition of pregnancy planning based on differentiated consideration of anatomic and functional changes of uterine tubes and level of adhesion process intensity of pelvis minor organs. Application of this method raises the frequency of spontaneous uterus pregnancy by two times, ensures faster renewal of physiological functions of the body in the postoperational period, reduces the frequency of postoperational complications and adhesion formation by 6.3% and 25% accordingly and improves the quality of life.
