Work was devoted to the improvement of results of surgical treatment of sub- and decompensation forms of pyloroduodenal stenosis of ulcerous etiology by development and application of method of selective proximal-vagotomy with gastropexy and drainage operation of stomach,with application of modern preoperative preparation and rehabilitation of patients in an early post operational period. To study more deeply on metabolism of patient in this category, the exchange process of lipid peroxide contributed to the development of different treatment tactics corresponding to the stage of ulcer pyloro duodenal stenosis and morphological pecularities of the ulcerative process The principal of preoperational preparation is based on a new classification which was developed from the information obtained from the study of morphological structure of pyloroduodenal area and the clinics of ulcerative pyloro duodenal stenosis Based on the above principles, resection of stomach and selective proximal vagotomy with gastropexy is proposed. This method also have been applied to improve the motorevacuation function of the stomach and gastrostasis. The usage of modern methods of antibacterial therapy, methods by the systemic and enteral disintoxication, and also applications of preparations of immuno-modulators action; increases the possibilities of using selective proximal vagotomy, and improves post operative results and prevents the post operative complications in works of clinical classification of ulcer pyloro duodenal stenosis ,with grounded to define for operative treatment; the new method of operation was developed at patients with sub- and decompensation ulcer pyloro duodenal stenosis, in basis of which selective proximal vagotomy, gastropexy and drainage operation of stomach lies with the maximally complete saving of pyloric sphincter.
