The dissertation work is dedicated to the research of age peculiarities, pathogenesis, diagnostics and treatment of the acute intestinal ischemia. It was studied deeply the biochemical markers of ischemia - reperfusion intestine failure and morphometric changes in small bowel mucosa. It was investigated the negative tendency of diagnostics and clinical course of the intestinal ischemia bowel failure. It was work out in experiment and introduced in clinical practice the selective regional angiomesenteric correction by the introduction of the correct medical composite with monitoring regional and systematic hemostasis. The dynamic sonographic monitoring of intestinal blood flow was introduced in clinical practice taking into considerance the level of occlusion and the failure extention. The comparable analysis of the results of surgical management of acute intestinal blood vessel insufficiency in elderly patients, made us to conclude that our diagnostic and management algorithm diminishes postoperative lethality on 15 % in segmental failure and 18,6 % in total intestinal blood vessel failure, comparing with traditional treatment patients. The level of local complications in segmental failure amounted 20 (83,3 %), in total failure amounted 8 (88,9 %), while systemic complications amounted 22 (91,7 %) patients with segmental failure and in all patients with total blood vessel intestine failure.
