Essay

The object is acute myocardial infarction with ST-segment elevation (STEMI), beta-adrenoblocators influence on the clinical course, parameters of endothelial function and system inflammation, heart rate variability (HRV) and myocardial depolarization/repolarization homogeneity. The aims to improve the treatment efficiency of STEМІ by studying nebivolol influence on the clinical course, as well as on the parameters of endothelial function and system inflammation, on HRV and myocardial depolarization/repolarization homogeneity. The methods are clinical, laboratory, instrumental and statistical methods. The new approach lies in establishing that disturbance of endothelial function, HRV, myocardial depolarization/repolarization homogeneity and system inflammation associate with complication of STEMI; determining that nebivolol exceeds metoprolol in antiarrythmic effect and influence on left ventricle systolic function; establishing that nebivolol is more effective than metoprolol in treating patients with anterior STEМІ, without reperfusion, with EF<40,0% at admission and with acute left ventricular failure Т. Killip I; ascertaining that nebivolol optimizes endothelial factors, markers of systemic inflammation, QT interval parameters and HRV. The field is medicine.
