Object of research: patients with essential hypertension at 1st and 2nd degrees. A purpose is optimization of treatment of hypertensive illness by application of the combined therapy of fozinoprilum and fozinoprilum H on the basis of establishment of changes of neyrogumoral'nikh factors, indexes of antikoagulation and fibrinolation links of the system of gemostaza, parameters of day's monitoring of arterial pressure of the system and vnutriserdechnoy gemodinamics, functional state of myocardium and indexes of endotelium disbalance. Methods of researches: the common clinical investigations, echocardiography, daily monitoring of blood pressure, biochemical methods of epinephrine, norepinephrine daily excretion research, protein C, antithrombin ІІІ, plasminogen's activity, contents of activation plazminogen-1 inhibitor, the test with vein occlusion and the test with vasoreactive hyperaemia. Novelty - connection is analysed between the simpatik-adrenalovium system and vazoaktiv function of humeral artery under act of treatment for patients hypertensive illness; the complex estimation of antihipertenziv effect of fozinoprilum and fozinoprilum H is done on the state of inwardly cardiac haemodinamik and diastole function of the left ventricle. Results of research: investigation of the echocardiographic indiced at patients with essential hypertension has shown reliable degradation of diastolic function in left ventricle, especially at its excentric hypertrophy. Carrying out of daily monitoring has allowed to constate the blood pressure daily profile disorders at increasing of systolic and diastolic arterial pressure average values, variability, the time hypertensions indexes and rates rising of morning blood pressure, especially at concentric hypertrophy. It was determined significant disorders of brachial artery vasomotor function, anticoagulant and fibrinolytic links indices of haemostasis changes. The followed six-monthly course of therapy by fosinopril and fosinopril Н was characterised by general peripheric vascular resistance decrease, left ventricle diastolic function enchancement, index, of the blood pressure variability and also anticoagulant and fibrinolitic links of haemostasis positive changes, veriable decrease of epinephrine and norepinephrine excretion. Results of work were introduced into 9th and 10th Zaporozhe city diversified clinical hospitals', Zaporozhe regional cardiologic dispensary's, Kakhovsky central regional hospital's of Kherson's region, Kirovograd regional hospital's treatment practice, and also into scientific and educational work of Zaporozhe medical academy of postgraduate education.
