Dissertation is devoted development of new operative treatment's methods of patients with ulcerous deseas of forestomach's back wall with the purpose of improvement of operative treatment's consequences. Basis of work was made by the results of 109 patient's inspection and treatment with ulcerous deseas of forestomach, which made basic (56 patients) and group of comparison (53 patients). The individual going is developed near the choice of operation allows to apply the most rational method for every concrete patient with the ulcer of forestomach. Set, that surgical interferences for patients with an enhanceable operating risk must be in the least degree traumatic and protracted, conducted in accordance with the severe observance of organ-saving principle and most physiologicaly. It is discovered that a degree of dystrophic and destructive changes of stomach wall round the crater of ulcer is depending on a volume. Submicroscopic changes after the depth and degree of expressed are circulating, after the removal of pathogenic factor which testifies to potential possibility of implementation of organ-saving interferences. Grounded the use of stomach's resection method wedge-shaped carving of ulcer on a back wall and proceeding in the physiology function of stomach with a maintainance its sphyncters. Application of the offered treatment and diagnostic algorithm of surgical tactic treatments in patients with the ulcers of forestomach, allows substantially to improve the results of surgical treatment and reduce afteroperative complications from 17,4 % до 4,6 %.
