The dissertation is devoted to the research of endocardial and system haemodynamics at old-aged patients with postinfarction cardiosclerosis and essential hypertension. The intercommunications between these two diseases, the age of the patients, the level of the blood pressure and the endothelial function have been well-proven. The recommendations for the effective treatment of such patients have been elucidated. On the basis of the integrated survey of the old-aged patients, who had had the heart-attack and suffered from the arterial hypertension, it has been determined that the postinfarction remodeling of the heart and the endothelial dysfunction progresses according to the degree of the arterial hypertension, age of the patient and functional class of the CHF. The close correlation between the main indexes of the systolic and diastolic heart functions, system haemodynamics and the level of Endotelin-1, final metabolites of nitric oxide are typical for the old-aged patients. That confirms the role of endothelial dysfunction in the development of the severe complications of IHD in combination with essential hypertension. The 12 weeks therapy with Carvedilol and Perindopril is accompanied by a reliable reduction of the blood pressure to the target level. It allows to improve not only the endothelial function of the vessels, but also the systolic and diastolic function of the left heart ventricular at old-aged patients with postinfarction cardiosclerosis in combination with essential hypertension.
