Object - patients with stable angina of functional class (FC) I-III, prior myocardial infarction, after surgical revascularization of myocardium and with chronic heart failure (CHF) of FC I-III. Purpose - to improve diagnosis and treatment of patients with ischemic heart disease (IHD) by means of examination of lipid metabolism, haemostasis, clinical parameters, structure and function of the heart, to discover their prognostic value for development of acute cardiovascular events. Methods - clinical, laboratory, instrumental, statistical. Novelty - for the first time, it was revealed activation of blood coagulation according to levels of fibrin monomer (FM) above 4 mg/dl in patients with total cholesterol (TC) levels <5,2 mmol/l, CHF of FC III and increased left heart chambers dimensions; it was proved that statins decreases activity of thrombogenesis by lowering of FM levels; it was revealed associations between hypercoagulation and dyslipidemia, changes of heart structure, CHF of FC III; it was proved that the FM level above 4 mg/dl is the most informative marker of prothrombotic disorders of haemostasis; it was showed that unfavourable prognosis in patients with TC levels <5,2 mmol/l occurs due to hypercoagulation and systolic dysfunction of the heart. Results - assessment of blood lipids and haemostasis parameters in patients with IHD and their changes during aspirin and statin therapy allows to optimize of IHD diagnosis and treatment; predictors of acute cardiovascular events were elaborated; results were implemented into clinical practice, a scientific and educational process.
