The dissertation is devoted to improving the effectiveness of treatment of patients with acute coronary syndrome without ST segment elevation with biochemical resistance to aspirin. According to survey of 465 patients with acute coronary syndrome without ST segment elevation established belonging of previous long receiving of aspirin at last 6 months to the additional risk factors for severe ischemic "events". Analyzed prevalence of resistance to aspirin among patients with acute coronary syndrome without ST segment elevation, who took aspirin regularly, and show its association with increased hospital mortality level, frequency of nonfatal (re) myocardial infarction and total frequency of these events. Describe the factors that increase the probability of resistance to aspirin (age older than 50 years, female, long reception aspirin more than 5 years, a history of existing myocardial infarction). Compared of values of platelet aggregation in patients with acute coronary syndrome and biochemical resistance to aspirin and without it. Consider the results of addition of clopidogrel to aspirin therapy in aspirin resistant patients.
