Yevtushenko D. Prevention for the development of impossibility sutures under operative treatment of patients on complicated by bleeding duodenum ulcer

The Thesis is dedicated to prevention for the development of impossibility sutures under operating treatment by complicated bleeding to ulcer disease of the duodenum. Materials of the department of acute gastrointestinal bleedings GI "Institute of general and urgent surgery" AMS Ukraine are used In work. According to the to purpose treatment 102 patient of the duodenum ulcer, complicated by bleeding was carried out. On the basis of carried out analysis of the clinical material of the comparative group of patients were established principal important aspects of the surgical treatment of ulcer disease of the duodenum, complicated by bleeding, which gave the development of new type of suture, new type of duodenoplasty and development of algorithm of surgical treatment. The Certain algorithm has allowed more rationally individualized methods of surgical treatment. The Particular reparative process at use of combined (the single-in-line extramucouse inverted precision) suture by polifilament syntetic thread of the gradual period of dissolution ("Vikril", dekson, makson) in period from 7 till 30th postoperative days comparable with use of dual in-line suture, turned out thet: double smaller percent of the anastomosis development, absence of erosion-ulcerous complications, full-fledged colagenesis of granulation tissue, more quick rates of the forming connectivtissue scar, as well as early and full-fledged regeneration of the epithelial cover. Thereby, the use of our treatment tactics surgical, allows to substantionally improve its favorable effect at patients with ulcer of the duodenum, complicated by bleeding.
