The thesis presents theoretical and practical reasons for the decision the actual problem of modern surgery, aimed to improve the treatment results of patients with purulent necrotic changes in the diabetic foot. The duration of the formation of a purulent process associated with a form of diabetic foot.The criteria for predicting the course of wound process interaction are: localization of suppurative focus, timing cleansing the wound surface and the appearance of repair in tissues, the spectrum of microflora and the degree of contamination of tissues. Using a modified classification of Wagner, has allowed to allocate the form of purulent-necrotic process, examine the wound process in every form, to highlight the adverse forms of purulent process, as well as to develop methods of access and drainage of the purulent focus. Based on the analysis cytograms, microbiological research, studying the severity of SIRS at each stage of purulent-necrotic process, highlighted the severity of wound healing: light, middle room severity, severe and very severe, with the loss of limbs. Given the severity of wound healing process, the algorithm of treatment aimed at all the links in the pathogenesis of the disease. An integrated approach to the treatment of patients with purulent-necrotic changes in the diabetic foot, reduces the number of repeat surgical intervention, reduces wound healing time, increases the number of patients with primary healing of wounds, reduces the number of high-limb amputation.
